Varicella Zoster Meningitis with Isolated Headache in an Immunocompetent Adult

Abstract

|
Varicella zoster virus (VZV) can infect the central nervous system (CNS), and complications include meningitis and encephalitis in elderly or immunocompromised patients. In CNS VZV infection, high fever and skin eruption are key diagnostic hallmarks, and diagnosis is confirmed with cerebrospinal fluid (CSF) analysis. We report a case of a 40-year-old healthy man who presented with a headache but no fever or skin eruption. CSF examination confirmed aseptic meningitis, and VZV DNA was detected on CSF polymerase chain reaction. In patients without any other CNS infectious signs or a skin eruption, VZV meningitis may be difficult to diagnose, which may lead to a delay in treatment. When headache treatments do not work by common medication, physicians should consider VZV meningitis at least once though there were no fever or skin eruptions.
